  • Save for Later- Click this button to save your current progress
    • Your answers are securely saved
    • You can return anytime to continue where you left off
    • The task remains "in-progress" until you submit
  • Automatic saving - Your progress may be automatically saved as you work
  • Returning to continue - Access the task from your dashboard or task list to resume

When to save for later: If the questionnaire is long, you're interrupted, or you need time to gather information for certain questions.

 

Submitting Your Completed Questionnaire

When you're ready to finish:

  1. Review your answers - Scroll through to check your responses
  2. Ensure all required questions are answered - The system will alert you if anything is missing
  3. Click the Submit button - This sends your completed questionnaire to your provider
  4. Wait for confirmation - You'll see a confirmation that your submission was successful
  5. Task status updates - The task will show as "completed" in your task list

After submission: Your responses are immediately available to your healthcare provider and cannot be changed. If you need to make corrections, contact your provider's office.
