Submitting Your Completed Questionnaire
When you're ready to finish:

- Click the Submit button - This sends your completed questionnaire to your provider
- Wait for confirmation - You'll see a confirmation that your submission was successful
- Task status updates - The task will show as "completed" in your task list
After submission: Your responses are immediately available to your healthcare provider and cannot be changed. If you need to make corrections, contact your provider's office.
